Heart disease doesn’t discriminate based on gender. But treatment sometimes does. Men are more likely to receive life-saving cardiac procedures, such as use of combination pacemaker-defibrillators.

Research released Monday by LDS Hospital and Cleveland Clinic, the two lead institutions on a 130-facility international study of how women fare with the device compared to men, says there’s no reason for the bias. Women do just as well.

“Hopefully, studies like this will show that if women qualify for these life-saving therapies, they do just as well,” said Dr. John Day, an interventional cardiologist at LDS Hospital and one of the lead investigators, with Dr. David Martin of Cleveland Clinic. “It’s hard to know why they’re underutilized in women versus men.”

The study looked at special combination pacemaker defibrillators designed for use by heart failure patients. The three-lead devices (most combo units have two leads) are connected to the heart and have been shown to improve heart function, increase exercise tolerance, decrease need for doctor visits and more, Day said.

In trying to resolve the gender-bias question, they looked at everything they could think of, he said. “Is there a difference in outcomes? Do women have more complications or are the surgeries more complex? Are they stuck in the hospital longer or is the device harder to install? We tried to look at all of the questions.”

Between December 2004 and November 2006, they looked at differences in outcome for 1,268 men and women who had the heart- failure pacemaker defibrillators implanted. One-third of the patients with the implanted devices were women.

They found no difference at all.

“Women did just as well, with no increased complications,” said Day.

The device is implanted just under the skin near the shoulder, then the leads are inserted into the vein and attached to the heart muscle. The pacemaker helps the heart keep pace and strengthens it. Should the heart stop beating, the defibrillator automatically shocks the heart to restore rhythm. It can also be triggered deliberately if needed.

That happened to Salt Lake City resident Marion Payne, who has had one of the devices since August. She’d suffered long bouts of atrial fibrillation. She underwent an ablation to “short circuit” the section of the heart’s wiring that was going awry. But after she had pneumonia last May, she again had atrial fibrillation and was implanted with the pacemaker-defibrillator a few weeks later. It’s never shocked her on its own, but her doctor activated the defibrillator function once to restore her heart rhythm.

She’s had no problems or complications with the device, she said.

Time has flown for fans of Carrie Bradshaw and her three gal- pals on the hit television series Sex and the City, which aired its last original episode on the HBO cable channel in the US in 2004. Their patience for a long-promised reprise on the big screen may have been about to run out.

But, according to a report in Variety, the wait may soon be over. The Hollywood trade paper contends that a deal has been signed between HBO and New Line Studios to begin shooting a film version in this autumn. New Line has yet to comment, but all four female leads - Sarah Jessica Parker, Kim Cattrall, Cynthia Nixon and Kristin Davis, who played Carrie Bradshaw, Samantha Jones, Miranda Hobbes and Charlotte York respectively - have supposedly agreed to the big- screen reunion.

There is no word, so far, on which of their many male suitors and partners may return, although Chris Noth, who played Carrie’s heartbreaker Mr Big, is also expected to appear, shortly before the closing credits.

Reviving the Manhattan antics of Carrie, with her wardrobe of Manolo Blahnik shoes and designer dresses, reportedly took s o long because of contract disputes, specifically with Cattrall.

Celebrity websites have long speculated that she was holding out to be paid on a par with Parker, who got more for the television show because of her role as executive producer. Variety also reported that as part of the film deal, Cattrall has been assured future roles in forthcoming HBO programming.

Parker, who won an Emmy award as Carrie, tried to play down rumours of a rift between the actresses in an interview when she launched her clothing line, Bitten, earlier this year. “It is a very complicated puzzle to put back but not for the reason that people speculate about, because as far as I know, all of the actresses want to be together again for this movie,” she said. Asked about a film, she replied: “Never say never.”

Most of the original production team are also expected to reunite for the film. The executive producer of the television show, Patrick King, has already written the film’s script. The series producer, John Melfi, and its creator, Darren Starr, have reportedly also signed on for the New Line project.

Sex and the City was originally inspired by a newspaper column by Candace Bushnell about her adventures dating and romancing in Manhattan.

First aired in 1998, Sex and the City quickly became a hit for HBO which, like New Line, is owned by Time Warner. It allowed it to emerge from the shadows of the traditional network broadcasters as a company able to produce first-rate comedy and drama. It followed up soon afterwards with its other stand-out success, The Sopranos, an award-winning drama series about a dysfunctional Mafia family in New Jersey.

The Court of Appeals considered Tuesday whether a Montgomery County judge was right to dismiss child sex abuse charges against an immigrant when the circuit court had trouble finding an interpreter.

Circuit Court Judge Katherine D. Savage drew furious criticism last July when she dismissed charges against Mahamu F. Kanneh, finding a violation of his right to a speedy trial. Almost three years passed between Kanneh’s arrest and the dismissal, with much of the delay due to the court having trouble finding someone who could speak and translate the proceedings in Kanneh’s native language, a Liberian dialect called Vai.

Hearing the state’s appeal of the dismissal, retired Judge Dale R. Cathell seemed frustrated with the idea of waiting indefinitely for a translator, but also with the idea of an alleged criminal going free.

“What happens when it’s impossible, period, to find an interpreter for some vague dialect in the middle of the Amazon jungle?” Cathell said. “What happens, the case stays in limbo until someone else emigrates from that part of Brazil?”

“If you happen to be from that particular tribe in the middle of the Amazon jungle you commit a crime and you can never be tried for it because they can’t get an interpreter for you?” said Cathell, who was on the bench by special assignment until his successor is named.

Kanneh, who was accused of repeatedly sexually assaulting a 7- year-old girl, has been ordered to be deported to Liberia, but that country has not yet agreed to take him back, Assistant Attorney General Kathryn Grill Graeff said. He is in the custody of U.S. Immigration and Customs Enforcement.

Applying Barker

Graeff argued that Savage misapplied the Supreme Court’s test for determining whether a defendant’s Sixth Amendment right to a speedy trial has been violated. That test, established in 1972’s Barker v. Wingo, requires a judge to consider the length of the delay, the reason for the delay, whether the defendant asserted his right to a speedy trial and any prejudice to the defendant.

Graeff argued that the reasons for the delay, including the interpreter issue and the need to obtain DNA testing, should not count against the state. First of all, she argued, it is the court’s responsibility, not the prosecutor’s, to find an interpreter. Secondly, everyone involved in the case made serious efforts to find someone who could translate, she said.

“I’d point out that as hard as it is to find a Vai interpreter and all the extensive effort, this isn’t an issue where, a case where there’s three years without an interpreter,” Graeff said.

The first Vai interpreter found the subject matter of the case disturbing and ran out of the courtroom; she also was unable to do simultaneous interpretation. The second interpreter had just undergone surgery, was in pain and did not know if she could sit through the trial. A third interpreter was present when Savage dismissed the charges.

Many of the appellate judges seemed skeptical that Kanneh really needed an interpreter in the first place, given that he has lived in the United States for years and attended high school here. Graeff, however, said that Kanneh’s attorney argued at the circuit court level that Kanneh could understand basic English but did not know enough to help with his own defense; the prosecutor wanted to protect Kanneh’s rights and so made every attempt to accommodate his request, Graeff said.

Though Savage stated when she granted Kanneh’s motion to dismiss the charges that the efforts to get an interpreter had been “Herculean,” Assistant Public Defender Amy E. Brennan disagreed.

“I would suggest to the court that what we have in terms of the court’s efforts, the timing of the efforts, the record does not support that there was a diligent pursuit of a translator or an interpreter from the get-go…” Brennan said.

The court knew from Kanneh’s first appearance with counsel that he needed an interpreter but did not take all the steps it should have to find one, she said.

Finding prejudice

Judge Joseph F. Murphy Jr. suggested that Kanneh’s lawyer could have proceeded without an interpreter.

“There’s one way to deal with it,” Murphy said. “Defense counsel could have said, ‘I object; we want to be tried. I object to the fact that he’s being tried without an interpreter and if he gets convicted I’m going to raise this issue with the appellate court and seek a new trial on his behalf. But we really want this case tried so we’re going to go forward.’”

“I can’t imagine wanting to go forward that desperately when your client doesn’t understand a thing that’s going on,” Brennan argued.

Graeff said there was also a delay caused by the state’s decision to consolidate Kanneh’s case with that of his father, accused of abusing the same child, and a second delay caused by the court severing the cases again. But, she said, those delays were only a matter of weeks.

Also, Kanneh waived his right to a trial within 180 days early in the case, only reasserting his right to a speedy trial a few months before the case was dismissed, Graeff pointed out. She also said Kanneh’s defense lawyer did not argue any actual prejudice to Kanneh’s defense because of the delay, just that Kanneh suffered anxiety and had to move out of his home because he was no longer allowed to be around children.

Criminal charges have been filed against an excommunicated general authority of The Church of Jesus Christ of Latter-day Saints.

Washington County prosecutors charged George Patrick Lee, 64, on Monday with a single count of failure to register as a sex offender, a class A misdemeanor. He made a brief appearance in St. George’s 5th District Court and was released from the Purgatory Jail after posting $5,000 bail. He will be arraigned on Aug. 2.

Washington County sheriff’s deputies got a tip that Lee had been convicted of child sex abuse in the past but had not registered as a sex offender, as he is required to do by law.

“Detective Sergeant (Shauna) Jones contacted the Utah State Sex Offender Registry and confirmed that George was required to register with the state of Utah and had not since 2001,” Washington County sheriff’s deputy Nolan Gray said in a probable cause statement filed with the criminal charges.

Deputies went to Lee’s Washington City apartment and arrested him on Friday.

In 1989, Lee was a member of the LDS Church’s First Quorum of the Seventy when he was excommunicated for “apostasy and other conduct unbecoming a member of the church.” LDS Church leaders offered no more of an explanation for the excommunication.

According to published accounts from the time in LDS Church News, Lee himself criticized the LDS Church for what he said was a misinterpretation of scriptures and the faith’s neglect of Native American issues.

In 1994, Lee pleaded guilty to a third-degree felony charge of attempted sex abuse of a child and sentenced to 18 months probation.

Lee is the fourth man to be apprehended since the Washington County Sheriff’s Office announced a crackdown on unregistered sex offenders in the Dixie area. Using a publicly accessible database, police agencies have been able to provide maps of where sex offenders live. That Web site is www.familywatchdog.us.

WILD child Emily Parr’s parents told last night how they have banned her from stripping off or having sex on Big Brother.

The feisty drama student has taken tons of bikinis into the house but Mandy and Roger Parr laid down some ground rules before she went in.

Mandy said: “I told Emily she’s forbidden from taking her top off. She’s not built to be a glamour model and she’s a talented actress so that’s not what she wants to be known for.

“I’ve told her if there are boys she likes in the house she’s not to do anything with them.”

Even twin sisters Jessica and Alice have told 19-year-old Emily to keep it clean. Jessica, 18, said: “She’s definitely not allowed to have sex on Big Brother - we’ve warned her.” Mandy, 44, who has told Emily even kissing is banned, added: “It’ll be interesting to see what she’s like with Ziggy, but he’s too muscly to be her type.

“I’d never have suggested she go on a programme like this but she’s always done what she wants to do. She has to be the centre of attention - there’s no stopping her.”

Emily is no stranger to the camera - she and her sisters had parts as dancers in Havana Nights: Dirty Dancing 2 and she’s been in Casualty. And the Peaches Geldof lookalike is due to appear in new Johnny Depp film Sweeney Todd, in which - ironically - her character is locked in a madhouse.

Mandy said at their Bristol home: “She got to meet Johnny Depp and came back in love with him, saying how sexy he was.”

She added: “I couldn’t believe it when I saw the headlines about my daughter being some sort of cocaine-taking lesbian.

“I don’t think she would do cocaine because Emily’s the type of girl who has to be in control. She always has to be in charge - she’s a nightmare.” Emily’s mum admitted that the student at the South West Academy of Dramatic Arts had taken a comforter in to the house with her for when it gets tough.

Mandy said: “She has a rag from an old T-shirt that she rubs against her nose to remind her of home.”

Emily has claimed she and the twins are all alike, the “Hilton sisters of Bristol” - like hotel heiress Paris they have a lap-dog. But Alice said: “I can’t believe she said that - we’re all so different. But I hope people like her - she’s great fun.

“One problem in the house might be that she’s so messy - it drives me mad. And she can’t cook at all. One day she actually asked me ‘How do you fry an egg?’”

Roger, 49, who works for a chemicals firm, reckons his daughter is in it to win it.

He said: “She’s calculating and likes to win, so I think she will have a game plan. She’ll befriend the other housemates but I think she’ll be sizing them up as well.”

A Palo Alto teenager testified in court Wednesday that she endured a sexual assault last October that was so painful she “really wanted to die.”

In her testimony during the preliminary hearing for defendant Todd David Burpee — the 2006 Palo Alto High School graduate accused of assaulting her — the teenager described being attacked, choked, smashed on the sidewalk, dragged into a car and sexually assaulted before she managed to escape.

Burpee, 20, pleaded not guilty to charges of attempted murder and sexual assault when he appeared before Judge Thang Nguyen Barrett in Santa Clara County Superior Court in January.

Dressed in a pink sweat shirt, blue jeans and pink sneakers, the teenager — identified only as Jane Doe — slumped in her seat in court and stared at the floor, a baseball cap obscuring her face.

She confirmed details presented in court of the Oct. 30 incident, in which a man waiting outside her apartment building attacked her from behind — after she had opened a door for him, then returned to her bicycle because she was scared.

The attacker covered her mouth with his hand, then pulled her to the ground and twisted her neck until she lost consciousness, the teenager testified.

When Deputy District Attorney James Leonard asked her how much the attack hurt, the teen said she “really wanted to die because it was so painful.”

The attacker later dragged her into a car, where she lay face down, covered by a sheet, as they drove off, she said. While she was in the car, the suspect was on the phone often and stopped the car several times, she said.

At one point, he got into the backseat with her, took off her belt and pants, and sexually assaulted her, the teenager said. When he parked the car in Sunnyvale and walked away, she fled, still wearing her backpack.

In a cross-examination, defense attorney Daniel Olmos asked the teenager whether it was possible her pants had been removed before, when she was being dragged to the car.

But she rejected the idea, noting that she had been wearing “skinny jeans,” a tight-fitting, tapered style of pants, as well as a belt.

“I’m pretty skinny, so every (pair of) pants I wear are too big, so I need to wear a belt,” she said.

When Leonard asked the teenager if she could identify her attacker inside the courtroom, she quickly glanced up, but said she was not sure.

Burpee, sitting next to his lawyer, showed little emotion during the nearly three-hour hearing.

His girlfriend, Sara Dahlen, declined to answer questions by invoking the Fifth Amendment protection against self-incrimination.

Other witnesses Wednesday included Susana Naranjo and Hector Chavez, Sunnyvale gardeners who found the teenager’s cell phone as well as paper images of naked people in a parking lot they had been cleaning.

Palo Alto police Officer April Wagner described the teenager’s injuries, which she observed and photographed at Santa Clara Valley Medical Center both on the night of the alleged assault as well as two days later.

A partial list of the injuries included a laceration that went entirely through the victim’s bottom lip; a head injury; facial swelling; pain in her jaw, stomach and upper teeth; dried blood on her face; broken blood vessels stretching from her ears to her neck; and abrasions to her nose, chin and hip bones.

Wagner said the nurse who performed the “sexual assault response team” examination said the teenager also suffered a half-inch laceration in her genital area.

Palo Alto police Sgt. Sandra Brown described the gold Oldsmobile she searched following Burpee’s arrest. It contained plastic bags, a white shoebox and a coat hanger — details that matched the teenager’s description of the back seat area in which she said she had been stashed.

Brown said the car also contained several CDs, and magazines and computer printouts with pornographic content — including one magazine titled “Asian Fever” — and a pay stub from Macy’s made out to Burpee.

The preliminary hearing will continue at 2:30 p.m. today.

Novelist Josephine Cox worked in a vinegar-bottling factory

In her first job, Josephine Cox used to put her feet up.

“I left school at 14 and got a job in a vinegar-bottling factory - well, shed - on the banks of the canal in Blackburn. It had green doors, like the ones in hospitals that they push stretchers through, with a 1ft gap at the bottom. The rats would run in from the canal, so we sat with our feet up on the chair.”

One woman would put a bottle on the carousel, which would turn so that the next could fill it with vinegar, after which the third would jam on the cap and finally Cox would slap on the label. She found it fascinating: not so much the work as the fellow workers.

“They were lovely, very down-to-earth women, in their fifties. They all looked like my Aunty Rosie.” And they talked, but perhaps more frankly than an aunt to a niece. “I was learning about life and sex.” They showed each other their bras and laughed about their husbands in bed. They sound like the sort of characters who might come out of her warm-hearted novels. This is because some of them do.

Cox used one of her workers as the basis of a character in her first book, Angels Cry Sometimes, and another in her novel based on her own life, Her Father’s Sins. In Let Loose the Tigers, outrageous women debag Tommy Trundler and shove him down a chute; he is based on the son who with his father used to come in reluctantly every day to run the ratty enterprise and be harassed by the female workers. Fortunately, the trousers-and-chute bit is fiction but the merciless whistling and winking was only too real.

“Tommy” never complained about being ridiculed but then people don’t, not even the nymphomaniac - this wasn’t one of the vinegary ladies - described so accurately in another novel that the neighbours knew exactly who it was. Thanks to a wonderful teacher who used to tell her stories and lend her books, Cox had always wanted to be writer: here was her raw material on a plate or, rather, in a bottle.

The work was mechanical and indeed must by now be completely mechanised. Yet Josephine believes it suited the fifty-something women: “It was a good job and paid well. They would have stayed there until they fell off their perches.”

Cox lasted only six months, at which point her parents separated and she moved south with her mother and nine siblings. Cox then found work in a plastic-mac factory, where she did the belts. “It was really posh - no rats.”
